Please provide these documents when submitting the admission form.
- Copy of child’s Birth Certificate (hard copy for checking)
- Copy of Immunization Record (hard copy for checking)
- One Child’s photos (excluding the one on application form), write name at the back. size : 1.5 inch X 2 inch
- Four envelopes with child’s name, address and $2.2 stamp
- Application fees $40
